Section 4- Liability Waiver & Assumption of Risk
I understand that participation in sport and physical activity, including pickleball, involves inherent of risks of injury. By signing below, I voluntarily assume all risks associated with my child participation in the OneMore Youth Program at OneMore Pickleball Club.

I hereby release and hold harmless OneMore Pickleball Club, its owners, staff, coaches, volunteers, and affiliates from any and all liability, claims, or demands for injuries, damages, or losses that may arise out of or in connection with my child's participation, except in cases of gross negligence or willful misconduct.

I acknowledge that it is my responsibility to inform ONEMORE staff of any medical or behavioral conditions that may affect my child(ren)'s safe participation.

Section 5- Photo / Video Consent
ONEMORE occasionally takes photos and short video clips during programs for the purpose ofsharing updates with families and promoting our programs on our website, social media, and printed materials.

Section 6 - Youth Program Policies (Summary)

Respect coaches, staff, and other players at all times.
Follow all safety instructions on and off the court.
Wear closed-toe athletic shoes and appropriate sports clothing.
No running on wet surfaces or in non-court areas.
Equipment must be used as instructed and returned after use.
Parents/guardians must check children in and out at the front desk.

ONEMORE reserves the right to pause or terminate participation if a child's behavior creates safety concerns.

Section 7 – Attendance & Behavior Guidelines
Attendance: Regular attendance helps your child progress and supports group safety.

Please arrive 5–10 minutes before the start of the session.
Notify ONEMORE in advance if your child will be absent.
Repeated unexcused absences may affect program placement or eligibility.

Behavior: Our goal is a safe, positive environment for all children.

We use positive redirection and encouragement whenever possible.
Unsafe, aggressive, or consistently disruptive behavior may result in temporary or permanent removal from the program to protect other participants.

Section 8 - Pricing & Discounts Acknowledgment
Standard Rates:

$30 per hr/session

Discounts:

10% off each additional sibling enrolled.

Billing & Payment

Families may choose to pay per session at the standard rate of $30 per session.
Families who enroll in the monthly program must pay the full monthly fee in advance to receive the discounted rate of $90 for four sessions (regular value $120).
Monthly payments are due on the first Monday of each month and will be processed on that day.
Payments are due prior to or at the time of service unless otherwise arranged.
Missed sessions are generally non-refundable unless cancelled by ONEMORE Pickleball Club.

Section 9 - Step Up for Students Instructions
If you are using Step Up For Students scholarship funds for this program:

Log in to your Step Up For Students parent portal.
Select ONEMORE Pickleball Club as the approved Physical Education service provider.
Approve the funding request for each child you are enrolling, as communicated by ONEMORE.
Ensure your child(ren) attend their scheduled sessions so we can accurately document services provided.
Respond promptly to any Step Up For Students notifications requesting approval of invoices or funding requests.

Section 10 - Final Authorization & Agreement
By signing below, I confirm that the information provided in this packet is accurate to the best of my knowledge. I acknowledge that I have read and agree to the Liability Waiver, Photo/Video Consent selection, Youth Program Policies, Attendance & Behavior Guidelines, Pricing & Discounts Acknowledgment, and Step Up for Students Instructions (if applicable)
